Library Barcode Activation Form:

Your Library barcode, found on the back of your CSS ID card, is your ticket to accessing Library services and materials. You will need it for such things as using the Library's databases, checking out books or videos, submitting Interlibrary Loan requests, or requesting materials through Rover.

Activating your card is a simple process. To do so, please fill out the following form completely.
